When you call or come to visit we will conduct an assessment to determine the appropriate level of care for your situation.
We will ask for information such as:
- Basic demographic information
- History of use and substances
- Health history information
- Details about what prompted you to call
- Previous treatment history
- Insurance information in order to check for coverage
- Other payment options for treatment
Touring the facility for rehab in Utah and meeting with the staff is the best way to get to know the Cold Creek and our programs. By seeing it first-hand and meeting the staff you’ll be able to more clearly understand the treatment programs and ask questions.
With limited enrollment and the duration of our programs longer than most, we can take reservation payments in advance to hold a room. We also put a full financial plan in place prior to intake so that paying for treatment doesn’t become a distraction to the client while trying to work on recovery.
Clients completing a hospital detox, or flying in from out of town are safely transported to Cold Creek to complete the admission process. The program policies and procedures are reviewed and all consent forms and release information along with financial paperwork is discussed and any questions answered. Each client meets with our medical staff for an medical intake assessment. The client is assisted through the entire intake process by an intake counselor. The staff is also here to help with Family Medical Leave Act, short term disability and other paperwork and can communicate with a client’s personal doctor, job supervisor, attorney, or probation officer if necessary.
Many new clients arrive to substance abuse treatment with questions about health insurance benefits. They may be unsure about what their insurance will cover. Included in treatment are medical billing services. Our medical billing manager will coordinate throughout treatment and answer any questions regarding your insurance benefits and coverage.
